Evaluation of projects helps to determine how well the toll is being utilized, staff compliance with the tool, and identifies barriers to its use (Krizanova et al., 2019). As my project focuses on re-admissions, I will be utilizing the re-admission rate as a major factor to measure success. I will also utilize patient surveys assessing the patient’s knowledge of their disease process both before the start of implementation and at the end of the project, as well as assessing the level of their quality of life. I will also be utilizing nursing surveys to assess attitudes towards change management and ease of use of the tools. These surveys assist in assessing readiness for change and provide information on what needs to be addressed with staff to aid in the sustainment of the tool (Hower et al., 2019). Staff and patient satisfaction with the tool will also weigh heavily on if the change goes into sustainment or not (Lulejian & Cantor, 2018).
